Community Funding 2009-2010
Applications & Six/Twelve Month Evaluation Reports
Deadline - November 30, 2009 at 5:00 p.m.
The City of Goodyear will be accepting applications for its
Community Funding Program from September 1 through November 30. The
program provides direct monetary grants to agencies and organizations that
provide health and human services for Goodyear residents. For your convenience,
you may fill out an online
application after September 1 (link
active until deadline ONLY). Applications will be accepted through November
30, 2009 at 5:00 p.m. and the deadline will be strictly followed. Please
understand that late applications will not be accepted as the
application and announcement are available to the public via the website.
Please read this information packet carefully before filling
out the application to ensure you complete the requirements. The City of Goodyear now requires all applicants who are awarded a monetary grant to complete a
6-month and a 12-month evaluation report stating how the funds were used in
achieving your goals and objectives. The application process is as follows:

September 1 through November 30, 2009 at 5:00 p.m. - Application submittals.
o A technical
review of each application will be made to determine eligibility.
o Community
Funding Committee to review and draft recommendations.
o City
Council Worksession to discuss Committee recommendations.

Spring 2010 - Formal City Council action taken on funding
requests. Applications accepted by the City Council will be part of the FY 10-11
budget adopted by the City Council.

April/May 2010 - Award recipients notified by letter of
the amount approved by City Council. Funding should be received approximately
August 2010.
·
March 15, 2010 - Six
Month Evaluation Report due on grants awarded in 2008/2009.
·
September 1, 2010 - Twelve
Month Year-End Report due on grants awarded in 2008/2009.
